I understand that my athlete's enrollment will be cancelled when Zone Athletics receives this cancellation form. I understand that this form must be received by the 5th of the billing month and if it is not received by the 5th, I understand that I will be billed for the following month. If I change my mind, I must re-enroll my athlete in a class or have them put on a waiting list. I understand that space may only be available in a class other than the one that I am withdrawing my athlete from.
